I've seen this problem on more than one machine and don't have an explanation for it. I recently installed several maintenance tools on a friend's machine. Following the successful installs, I dragged the program icons into a desktop folder, at which point they look fine. A few hours later, after doing some other clean-up tasks, I noticed that several of the icons for the recently installed programs no longer show the expected program icon (eg. MalewareBytes, Revo, and a couple of others). Rather, they show the generic windows icon (page with a folded corner and a bunch of little colored "icon?" symbols). The affected shortcuts still launch their respective programs just fine.
If I open the Properties box for an affected shortcut, it shows the program's appropriate icon, both in the General tab and in the dialog for changing the icon. So, what's the fix? Nothing I've tried has managed to restore to appropriate program icon.
